I have used 9 conductor with foil shield and drain with the drain
soldered on to the case of the DB connector at one end only.  This will
shield and ground the cable but not allow it to be a ground loop and put
hum on the data wires.  24 gauge seems to be the standard that I find
around.  I like to make my cables all 9 pins straight through and built
as extenders so I can use them elsewhere if I want to later or with
different connections at either ends.  I then just use the stock short
cables that came with whatever hardware and the short cables have
whatever required pin changes are needed for that particular piece or
hardware.  If the ends don't match, I can use gender changers, which
most of us dealing with this type of system will probably have a few
stashed in a drawer or box.  With all 9 pins in the cable, I will never
have a problem of not enough wires for a particular piece of hardware
that wants more than the three wire send/receive/ground  that many
RS-232 devices only need.  If the hardware wants a DB25 connector, that
can also be adapted, with no problems to 9 conductors.
