Dan, great clarification - I've got the same arrangement here in Calif. I
would think that ALL breaker boxes are arranged that way, simply because
that's the only way you can install paired breakers for 220V devices.

Additionally, you can often (not completely reliably, of course) identify
the phases by the color of the wires. For me, one phase uses red, the other
uses black.

For this reason, you should also be VERY careful when replacing a single
breaker with a double one - it's quite likely that you are inadvertently
moving one circuit from one phase to the other. At best, that will mess up
the color coding. At worst, you may upset the balancing of the load between
the phases; the electrician who originally planned your house made sure
that the load on both phases is approximately equal.

Easy way to avoid that: skip a breaker. If you replace one single breaker
with a double, remove not the breaker directly below or above it, but one
that is two spaces above or below.
