I found a pretty good example in NT DDK this morning.
I simply modified it a little, and I'm sending you a small attachment
containing the device driver (.SYS), install info and two sources
showing how to perform I/O.
At the moment it will read/write a single port, a double port or 4-wide
port from address 0x300. This could be changed from the registry or the
INI file.
